Take the first step for 2014 benefits Open Enrollment

It’s time to take the first step for 2014 benefits Open Enrollment.

Starting today, the Personal Health Assessment (PHA) window is open. If enrolled in a medical plan, you and your spouse/domestic partner can log in and complete the PHA. The deadline to complete the PHA is October 31, 2014.

If you are enrolled in the PEBB Statewide or Providence Choice medical plan:

  • Go to myProvidence.org ,“Log in” or “Create an account” if you are new to the system, use your medical ID number.
  • Click “Go Now”
  • Click “Continue” to accept the privacy statement
  • Complete the questionnaire. The PHA requires answers to questions 2, 3, either 5 and 6 or 7 and 66.
  • Click “Submit Now”. Completion will NOT be counted until the “Submit Now” button is clicked.
  • Print the Completion Certificate for your records

If you are enrolled in a Kaiser medical plan:

  • Go to Healthworks by Kaiser Permanente, “Log in” or “Create an account” if you are new to the system, use your medical ID number.
  • Complete the questionnaire
  • Print the Completion Certificate for your records

If you elect not to complete the PHA, an additional $100 will be added to your 2015 individual deductible. If you elect to complete the PHA, you will receive an incentive of $17.50/month for you and your spouse or domestic partner. 

Watch for additional Open Enrollment information to be sent throughout September and October.
