COVID-19 Booster Reporting

The UO requires faculty, staff, and students to get a booster shot.

Deadline is approaching:
The deadline to meet the booster requirement is January 31, or 30 days after you become eligible. 

For Employees
Have your booster?
If you have not already done so, complete the vaccination reporting form.
  • You will provide all vaccination dates and upload the front and back image of your vaccination card.
  • In the booster section, upload the documentation you were given or your vaccination card again, if your booster was added to your original card. 
Not yet eligible?
Get your COVID-19 booster shot as soon as you are eligible and complete the vaccination reporting form within 30 days of your eligibility.

For Students

Report your COVID-19 booster the same way you provide other immunization information.
Log into myUOHealth, and go to the "immunization" tab. 
  • Not yet eligible?
    Get your COVID-19 booster shot as soon as you are eligible and complete the vaccination reporting form within 30 days of your eligibility.

  • Have an exemption?
    If you have received an exemption to the primary COVID-19 vaccination, no further action is needed. Your exemption record will apply to the booster.

Why are boosters important?

According to guidance from the CDC, though COVID-19 vaccines remain effective at preventing serious illness, their effectiveness to prevent milder illness decreases over time. Current data shows booster shots increase immune response to COVID-19. 
With the spread of highly transmissible variants, such as Omicron, booster shots are one tool for preventing the spread of COVID-19 on campus.

Where can I get a booster shot?

COVID-19 vaccines are available in the Eugene/Springfield community. Lane County Public Health is offering booster shot clinics and provides a list of additional places, including health care providers and pharmacies, that offer COVID-19 vaccinations. Walk-in vaccination clinics at PeaceHealth RiverBend Annex are now open Tuesday-Sunday. Appointments are not needed for the walk-in clinics.
If you are outside of the Lane County area, check with a local public health agency, pharmacies, or health care providers about booster shot options and appointments in your area.
