Booster Reporting: UO students, faculty, and staff are required to receive a COVID-19 booster when eligible. To submit your booster shot information:
- Students: Log in to the MyUOHealth Portal. Visit the "Immunizations" tab. This is the same process used to upload your original vaccination information.
- Employees: Fill out this online form and upload a copy of your vaccine card. This is the same form used to verify your initial vaccine status.
Exemption Process: As with our existing vaccination requirements, the University of Oregon will continue to recognize both medical and non-medical exemptions to the booster shot requirement consistent with state law. Details about the exemption process are available on the Student and Employee vaccination requirement webpages.
Booster shot information is due by January 31st or 30 days after you become eligible. For answers to common questions, visit the Coronavirus Resources website.
UO's Monitoring and Assessment Program (MAP) Increases Testing Appointments: To help with the community need for COVID-19 testing, MAP has nearly doubled daily testing appointments starting this week.
Individuals without symptoms can make appointments for testing inside McArthur Court. The MAP appointment schedule is available Wednesday afternoon for the following week.
Individuals with mild symptoms and not needing a medical evaluation can receive testing at the McArthur Court ticket booths, no appointment needed.
